False PHD is a passive item added to The Binding of Isaac: Rebirth in the Repentance DLC. It acts as the malevolent counterpart to the standard PHD item. Instead of turning all your pills positive, it converts good pills into bad pills while offering powerful combat benefits in return. Core Effects
Immediate Health: Spawns one Black Heart instantly upon pickup.
Full Identification: Automatically identifies the effects of all pills before you take them.
The Stat-Down Trade: Every time you consume a stat-decreasing pill, you suffer the normal stat loss but gain a permanent +0.6 Damage (+1.2 for Horse Pills). This effect applies retroactively to any bad pills you swallowed earlier in the run.
Black Heart Generation: Consuming a non-stat-down “bad” pill (such as Amnesia, Addicted, or Paralysis) forces the game to drop a Black Heart on the ground. Neutral pills like “X-Lax” or “I Found Pills” are unaffected and yield no bonuses. Strategic Synergy and Combos
While lowering your own stats sounds risky, certain item combinations can break the downside entirely:
Rock Bottom: This is the ultimate synergy. It locks your stats at their highest value, allowing you to absorb the raw +0.6 damage buffs from stat-down pills without actually losing any of your other stats.
Lucky Foot / Virgo / Regular PHD: Combining False PHD with these items neutralizes the pill-conversion penalty. Pills can spawn as both good and bad again, but you still keep the bonus damage and Black Hearts whenever you pop a bad one.
Placebo: Paired with a harmless effect like Paralysis or Amnesia, you can continuously duplicate the pill to farm infinite Black Hearts.
